Alcohol & Drug Evaluation?

A professional alcohol and drug evaluation is a comprehensive clinical assessment conducted by a certified professional to determine whether you have a substance use concern and, if so, what recommendations may be appropriate.

The evaluation looks at your history, current situation, and relevant factors that may affect your well-being. The goal is to provide a fair, evidence-informed opinion and clear recommendations tailored to your circumstances.

All evaluations are conducted with respect, confidentiality, and a commitment to helping you move forward.

Why a Professional Alcohol & Drug Evaluation May Be Needed

Legal and Court Matters

Evaluations are often ordered by the court as part of a case, diversion agreement, or sentencing requirement. The report provides the court with an objective assessment and recommendations.

School and DCFS Involvement

School and child welfare agencies may request an evaluation when substance use is a concern that could impact academic performance, attendance, or family functioning.

Workplace and Professional Requirements

Some employers and licensing boards require an evaluation when concerns arise or as part of a return-to-work or reinstatement process. The report helps decision-makers understand your situation and next steps.

Personal and Self-Directed Reasons

You do not need a court order to seek an evaluation. Many individuals choose to gain clarity, obtain recommendations, and take proactive steps toward their goals.

What to Expect

Documentation Review

We begin by reviewing any documents related to your evaluation, such as court orders, police reports, referral letters, or employer/school requests. This helps us understand the context and requirements.

Client Forms and Payment

You will receive client forms that include demographic information, service and fee agreements, client rights and responsibilities, required acknowledgements, and authorization forms. All client forms must be completed and returned, and the $300 evaluation fee must be paid in full before your appointment can be scheduled.

Two-Hour Evaluation

During your appointment, the evaluator will complete a comprehensive interview covering your alcohol and drug use, medical and mental health history, current circumstances, and other relevant areas. Standardized screening tools will also be completed as part of the assessment.

Corroborative Information

When appropriate and authorized, we may contact collateral sources (such as treatment providers, employers, probation officers, case workers, attorneys, or family members) to gather additional information relevant to the evaluation. This ensures the most accurate and well-informed recommendations. 

Your Findings and Written Report

We will review and discuss the findings and any recommendations with you before the report is finalized and released. The ensures you have the opportunity to ask questions and understand the recommendations.

If additional corroborative information is received after the interview, the recommendations may change. Written reports are typically completed within five days of having all needed information and released only to those you authorized.

What You'll Need Before Scheduling Your Alcohol & Drug Evaluation

Court, Legal, or DCFS

Provide your court order, referral letter, case number, attorney or probation officer contact information, and any deadlines. The ensures we meet all requirements.

Workplace or School

Provide the name and contact information of the requesting organization or licensing board, any forms they require, and deadlines, if applicable.

Personal Evaluation

No referral is needed. We will discuss your goals and make sure the evaluation is the right fit for your needs.

After you request an evaluation, we will review your documentation to ensure we understand the requirements and can complete the assessment.

You will receive secure links to complete the required client forms. Once your forms are returned, the office will contact you to collect payment and schedule your two-hour appointment.
